Description
ACTION AND MECHANISM
- [ANTIMETIC], [HISTAMINE H1 ANTAGONIST], [MUSCARINIC CHOLINERGIC ANTAGONIST]. Dimenhydrinate exerts an antiemetic effect probably due to antagonism of central histamine H1 and muscarinic receptors. Caffeine decreases the sedative effect of dimenhydrinate.
SPECIAL WARNINGS
- Dimenhydrinate may mask the ototoxic effects of some drugs; therefore, periodic assessment of auditory function is recommended in patients treated with these drugs. - Before using this medication in patients with vomiting of unknown origin, it is recommended to rule out appendicitis. - It is recommended to monitor the patient's hydration during a heat wave, especially if the patient is a young child, an elderly person, or a person with a serious illness. - Due to the anti-allergic effects of this medication, it may produce false negatives in skin tests for hypersensitivity to antigenic extracts. It is recommended to discontinue administration of this medication at least 72 hours before the test.
PATIENT ADVICE
- This medication will be administered with food or drink.
- It is advisable to administer dimenhydrinate at least half an hour before the trip, and continue the doses throughout the trip if necessary, respecting the recommended dosing intervals.
- Patients undergoing treatment with sedative drugs are not advised to self-medicate with dimenhydrinate products without consulting a doctor.
- It is advisable to avoid sun exposure during treatment.
- Any chronic conditions the patient has must be reported to the doctor before starting treatment.
- Exposure to extreme temperatures should be avoided during the use of this medication; stay in a cool environment and stay adequately hydrated.
This medication may negatively affect your ability to drive. Avoid driving until you are sure the medication does not significantly affect you.
CONTRAINDICATIONS
- Hypersensitivity to any component of the medication, including [XANTHINE ALLERGY].- [ASTHMATIC CRISIS].- [PORPHYRIA]. H1 antihistamines have been associated with the occurrence of porphyric attacks.
ADVANCED AGE
The elderly are more sensitive to the anticholinergic effects of antihistamines (such as dry mouth, urinary retention, glaucoma, confusion, and hypotension). Caution is advised.
EFFECTS ON DRIVING
- It may substantially impair the ability to drive because drowsiness or decreased reaction time is common, even at recommended doses. Patients should avoid driving until they are reasonably certain that the medication does not adversely affect them.
PREGNANCY
The safety and efficacy of this medication in pregnant women have not been evaluated. Its use is only recommended when safer therapeutic alternatives are unavailable.
INDICATIONS
- Treatment of [NAUSEA] and [VOMITING] associated with [DIZZINESS] in [MOTION SICKNESS].
- [VERTIGO].
INTERACTIONS
Dimenhydrinate may mask the symptoms induced by ototoxic drugs by reducing vertigo or dizziness. Similarly, it may potentiate the photosensitizing effects of other active ingredients that cause photosensitivity reactions. Furthermore, due to its activity, it may potentiate the effects of other sedative and anticholinergic drugs.
LACTATION
Dimenhydrinate is excreted in breast milk, so due to the risk of adverse reactions in the infant, it is recommended not to use this medicine.
CHILDREN
Children are more sensitive to the side effects of H1 antihistamines. Caution is advised.
GUIDELINES FOR PROPER ADMINISTRATION
It is recommended to take this medication preferably 1-2 hours before travel, and if this is not possible, at least 30 minutes before. Doses should be repeated as needed during the trip, respecting the recommended intervals.
POSOLOGY
- Adults: 1-2 tablets before travel. This dose should be repeated every 3-4 hours for the duration of the journey. If motion sickness has already occurred, 2 tablets should be taken at once.
- Children: The safety and efficacy of this medicine in children have not been evaluated.
PRECAUTIONS
- [RENAL INSUFFICIENCY]. Dimenhydrinate accumulation may occur because it and its metabolites are eliminated in the urine. - [HEPATIC INSUFFICIENCY]. Dimenhydrinate accumulation may occur due to its high hepatic metabolism. - Patients suffering from [GLAUCOMA], [PROSTATIC HYPERPLASIA] or [URINARY BLADDER OBSTRUCTION], [HYPERTENSION], [CARDIAC ARRHYTHMIA], [MYASTHENIA GRAVIS], stenosing [PEPTIC ULCER] or [INTESTINAL OBSTRUCTION]. Exacerbation may occur due to the anticholinergic effects of dimenhydrinate. - Diseases of the lower respiratory tract, such as [ASTHMA], [PULMONARY EMPHYSEMA] or [CHRONIC OBSTRUCTIVE PULMONARY DISEASE]. H1 antihistamines may thicken bronchial secretions due to their anticholinergic effects. [EPILEPSY]. Antihistamines have occasionally been associated with paradoxical hyperexcitability reactions. [APPENDICITIS]. Due to its antiemetic effects, it may delay the diagnosis of appendicitis. Ototoxicity. Dimenhydrinate may improve vertigo, tinnitus, and dizziness, thus potentially masking ototoxicity induced by ototoxic drugs. Photosensitivity. Dimenhydrinate may cause photosensitivity reactions; therefore, sun exposure should be avoided during treatment, and sunscreen should be used. Extreme temperatures. It may worsen heatstroke due to decreased sweating caused by its anticholinergic effects. Exposure to very high temperatures should be avoided, and appropriate hygiene and dietary measures, such as adequate ventilation and hydration, should be followed.
ADVERSE REACTIONS
Side effects are usually mild and transient, especially at the beginning of treatment. - Digestive: [NAUSEA], [VOMITING], [DRY MOUTH], [CONSTIPATION]. - Cardiovascular: [TACHYCARDIA]. - Neurological/psychological: [DROWSY]. [EXCITABILITY] may occur in young children and the elderly. - Ophthalmological: [BLURRED VISION] and [MYDRIASIS] may occur. - Allergic/dermatological: [HYPERSENSITIVITY REACTIONS], with [URTICARIA], [PRURITUS], [SKIN RASHES]. [PHOTOSENSITIVITY REACTIONS] may occur.
OVERDOSE
Due to its wide therapeutic margin, adverse reactions are only expected in the case of significant tablet ingestion. Central nervous system depression and anticholinergic symptoms may occur. Treatment will be symptomatic, along with standard elimination measures (see Dimenhydrinate).
